Bluetails Pet Rescue Adoption Application
Thank you for your interest in adopting from Bluetails Pet Rescue. Bluetails will adopt animals to adults (must be 21 or older) who demonstrate they are responsible individuals and can provide adequate assurances that they intend to provide proper care for an animal's entire life.
Please answer all questions completely and thoroughly. This includes landlord name & phone number & veterinarian name & phone number. We also recommend that you contact your landlord and vet and grant them permission to disclose your information to us. If you purchased heart-worm prevention someplace other than your veterinarian, you will be required to show proof. If we need to request additional information or wait for permission to be granted, we will move on to the next applicant while we are waiting for your information.
We strictly adhere to our adoption process as it is designed to find the best possible placement for each homeless animal in rescue. The basic steps are described on Our Adoption Process Page.
Please note that applications NOT completed online (downloadable version) will have a longer processing time.
Completing an application and/or paying the application fee does not guarantee final adoption approval. BLUETAILS reserves the right to deny any applications that we deem unfit or unsuitable for the animal.
